Lymphoedema is a chronic debilitating condition which can be the result of previous surgery for cancer where lymph nodes have been removed (breast cancer, melanoma, gynecological or urological cancers) or can develop from early age for unknown reasons (primary or congenital lymphoedema). Recent advances in microsurgery and new technologies have resulted in improved understanding of this disease process which has guided development of new surgical treatments for lymphoedema. DIEP FLAP BREAST RECONSTRUCTION WITH LYMPHOEDEMA TREATMENT OR PREVENTION
DIEP FLAP (Deep inferior epigastric artery perforator flap) is the mainstay procedure today to reconstruct the breast with patients’ own tissue. This complex technique, developed by dr Allen (New York, USA) has recently been upgraded with adding vascularised lymph node transplant to prevent lymphoedema in patients undergoing mastectomy and DIEP flap breast reconstruction by prof Masia (Barcelona, Spain). Same technique can be used for patients in need of breast reconstruction with already established lymphedema to their upper limb to treat it. Mr Kosutic is the only plastic surgeon in UK currently offering this technique of ‘total natural breast reconstruction’ with lymphoedema treatment or prevention, as he has been trained by both prof. Masia (completed Fellowship in breast reconstruction, perforator flaps and microsurgery) and dr Allen in New York. Patients requiring mastectomy can now benefit from both natural tissue breast reconstruction and prevention of lymphedema in the same procedure. Patients who previously had mastectomy, can now get their surgical lymphoedema treatment at the same time with DIEP surgery natural tissue breast reconstruction. Mr Damir Kosutic is internationally acclaimed expert in Lymphoedema Microsurgery. He is the only UK-practicing Consultant Plastic and Reconstructive Surgeon, who is fully trained and experienced in both vascularized free lymph-node transfer and lymphatico-venous anastomosis-super-microsurgery for treatment of lymphoedema. Mr Kosutic has been trained in these revolutionary techniques by prof. Ming-Huei Cheng, a pioneer in lymphoedeama microsurgery from Taiwan.
